In order to gain insight into potential organizational risk, a major public university system in southern United States recently conducted a survey to understand how faculty, staff and students reported potential issues across their various campuses. Specifically, the survey focused on what happened when someone observed a violation of the university’s policies, e.g. financial fraud, student safety, research impropriety, discrimination, and more.
Based on results from this study, university leaders noticed two critical weaknesses that exposed the whole system to an unacceptable level of risk
